  1. Physiology of Endocrine and diabetes disease:

Endocrine disorders can be classified according to the origin and the intensity of hormonal activity. From intensity of the secretion activity of ductless gland we are able to distinguish hyper functioning of ductless gland, which is characterized by increased secretion of its hormone and by increased concentration of this hormone in blood circulation. Hypo function of endocrine gland, which is characterized by decreased concentration of this hormone in circulating blood and decreased secretion of its hormone as well.

Endocrine illness results once an organ produces deficient or an excessive amount of associate endocrine secretion referred to as secretion imbalance.        2. Diabetes Types and its Complications: 

Diabetes, it is a condition in which the pancreas either produces too little insulin or none at all. The insulin can't be used effectively by the body. This allows blood glucose levels to rise while the rest of your cells are deprived of much-needed energy. This can lead to a condition in which every major system of body gets affected.

  • Type 1 diabetes: It is a chronic condition in which the pancreas produces little or no insulin. People with this type of diabetes, inject insulin on a daily basis to control the glucose levels.
  • Type 2 diabetes: It is chronic condition which affects the way the body processes blood sugar. It is a non-insulin adult-onset kind of diabetes, which accounts for at least 90% of the cases. The early stages of diabetes can be controlled by proper diet and exercise.
  • Insulin: It is a hormone produced in the pancreas by the islets of Langerhans, which helps in the regulation of glucose in the blood. Insulin helps in regulation of blood sugar level from getting too high or too low. The lack of insulin causes a form of Diabetic disease.
  • Hypo and Hyperglycemia: Hyperglycemia refers to chronically high blood glucose levels. Hypoglycemia refers to hazardously low blood glucose levels that drop below 70mg/dL. It is an acute complication of diabetes and occurs in individuals who use insulin.
  • Bone and Mineral Disorders: Bone and Mineral disorders occur when kidneys fail to maintain proper levels of calcium and phosphorus in the blood. They can be related to with endocrine disorders, chronic uropathy, nutritional deficiencies or metabolic disorders.
  • Blood Sugar: Blood Sugar refers to sugar that is transported through the blood stream to supply energy to all the cells in our body. This sugar is made from the food we eat.
  • Osteoporosis: It is a medical condition in which the bones become brittle and fragile from loss of tissue, mainly because of hormonal changes or deficiency of either calcium or vitamin D.

  1. Endocrine disorders:

Endocrine Disorders occurs when a gland produces too much or too little of an endocrine hormone, leading to hormonal imbalance and the development of lesions in the endocrine system, also affects the hormone levels.

  • Adrenal insufficiency: can be a condition at intervals that the adrenal glands don't manufacture adequate amounts of steroid hormones, primarily cortisol; however may embrace impaired production of adrenal cortical steroid(an aldosterone),which regulates sodium and potassium conservation and water retention. It is a disease which is also known as Addison’s disease. The major symptoms include Chronic or worsening fatigue, muscle weakness, loss of appetite, and weight loss. Nausea, vomiting, and diarrhoea are common in about 50% of cases.
  • Hypothyroidism: It is a condition in which the thyroid gland does not produce enough thyroid hormone. It is conjointly referred to as hypoactive thyroid or low thyroid. The low thyroid hormone in the body causes various heart related problems, fatigue, nerve injury, infertility, depression, constipation, dry skin etc. If left untreated, may lead to death.
  • Hypopituitarism: It is also unknown as pituitary insufficiency. It is condition in which the pituitary gland doesn’t release any hormones. It is interrelated to various diseases. Women with low pituitary hormone may cause irregular periods. In children, mainly it affects the growth hormone. If the growth levels are too high, it may lead to abnormal growth of the body and if the growth hormone is too less, it leads in the retardation of the child’s growth.
  • Multiple endocrine neoplasia: These rare, genetic conditions within the families. They mainly cause tumours of the adrenal, parathyroid and thyroid glands, leading to overproduction of the hormone.

  1. Diabetes Complications:

Diabetes is a disease which intensifies the danger of long-term complications. It increases the risk of cardiovascular diseases also known as diabetic cardiomyopathy. The chief complications of diabetes are due to the injury in the blood vessel which causes damage to nerves, eyes and kidneys. The damage to the eyes is known as diabetic retinopathy which is caused by damage to the blood vessels in the retina of eye resulting in diabetic eye complication. The damage to the kidney is known as diabetic nephropathy, which can pave the way to damaging tissue, urine protein damage, eventually leading to renal disorder or diabetic nephropathy. Almost two third of the people with diabetes may have signs of onset of kidney problems at early stages. Diabetes is also one of the main causes of Renal or kidney failure. The damage to the nerves of the body is known as diabetic neuropathy. It is one of the main complications of diabetes. The number of years of having diabetes is directly proportional to high risk of complications. The diabetic complications sometimes may be restricting or even lethal.   1. Diabetic Cardiomyopathy

There is a correlation between cardiovascular disease (CVD) and diabetes. Minimum 68% of people aging between 65 or older with diabetes die from heart disease; and 16% die of stroke. Diabetes is treatable disease, but even when blood sugar levels are under control it mostly increases the risk of heart disease and stroke. Because people with diabetes, particularly type 2 diabetes Mellitus, may have the conditions that contribute to their risk for developing cardiovascular disease such as:

  • High blood pressure (hypertension)
  • Abnormal cholesterol and high triglycerides
  • Obesity or overweight
  • Lack of physical activity
  • Poorly controlled blood sugars levels

  1. Diabetic Kidney disease or Diabetic Nephropathy:

Diabetic Nephropathy is a condition mainly caused due to diabetes affecting the kidneys. Sometimes, it may also lead to kidney or renal failure. The waste from the blood is eliminated by the blood vessels present in the kidney, which can also be destroyed by high blood sugar levels. The body usually retains excess water and salts leading to improper weight gain and swelling of ankle which may in turn leads to kidney or renal failure. In the earlier stages, no symptoms are seen. But the regular urine tests are required to detect the damage.

The high blood sugar levels may also lead in the damage of nerves, which in turn lead to difficulty in emptying of the urinary bladder. The long term retention of urine in the urinary bladder causes infectious bacterial growth with high blood sugar levels.

  1. Diabetic Eye disease or Diabetic Retinopathy:

Diabetic retinopathy is caused because of injury to the blood vessels within the tissue of tissue of retina. Poor management of glucose is one of the risk factors. The blood vessels might swell and leak fluid and abnormal development of new blood vessels on the surface of the tissue layer is seen. The related connective tissue may contract and detach from retina. Symptoms in initial stages embrace floaters, blurriness, dark areas of vision and difficulty in detecting colours. There is no cure for diabetic retinopathy however will be treated optical laser treatments and surgical removal of vitreous gel (vitrectomy) for good vision. There are four stages in diabetic retinopathy which are classified as mild, moderate and severe non-proliferative and proliferative. In the first stage, mild non-proliferative, there will be balloon-like swelling in blood vessels of the retina.

      8. Diabetic Gastro Paresis:

Gastro Paresis is also known as delayed gastric emptying. It is a condition which affects both type 1 and type 2 diabetes patients in which the stomach requires time to empty its contents. If glucose levels are high for a long period of time it may lead to the damage of vagus nerve. The vagus nerve has complete control in the movement of food through the GI tract. If the vagus nerve gets damage or doesn’t work properly, the muscle of the stomach and intestine gets blocked, and the movement of food is slowed or stopped completely.

  • Heartburn
  • Feeling of fullness while eating
  • Bloating of abdomen
  • Weight loss
  • Spasmodic stomach.
  • Constipation
  • Gastro Esophageal reflux.
  • Erratic blood glucose levels.

  1. Diabetic Foot and ankle:

Diabetic patients are mainly prone to have foot and ankle problems, which occurs due to complications such as nerve damage (neuropathy) and poor circulation of blood. Usually neuropathy can cause the loss of feeling in the foot, and it takes away the ability to feel discomfort and pain, or injury or any kind of irritation in the foot. The poor circulation of blood reduces the ability to heal, making it even hard for a small cut to resist infection.

The minor wounds like blister to foot may lead to serious consequences, as blood flow decreases in diabetic patients the wound healing is delayed leading to spreading of infection quickly. Chronic or previously treated wounds may lead to microbial cultures for which antimicrobial medical aid is needed.

Juvenile and adolescent diabetes is also known as Paediatric diabetes. It is a condition in children with prolonged endocrine disorder like physical growth, sexual development and high blood sugar (diabetes). In type 1 diabetes, the body loses its ability to produce insulin cause of destruction of beta cells so most of the people rely on exogenous insulin for lifetime. Mostly the children with type 1 diabetes are obese and with type 2 diabetes are at higher risk of cardiovascular disease and micro vascular complications.

  1. GDM (Gestational diabetes Mellitus):

Gestational diabetes is high blood sugar which develops during pregnancy and disappears once the child is delivered. During pregnancy, the placenta makes a hormone that helps in building up of glucose in the blood. The pancreas makes enough insulin to handle that. If not, the blood sugar levels will raise leading to gestational diabetes.

Gestational diabetes is usually detected in the second trimester of pregnancy. The doctor will check to see if you have gestational diabetes between 24 and 28 week of your pregnancy. The doctor may test sooner if you're at high risk of developing gestational diabetes. This may lead to difficulties during the delivery and increases the chances of caesarean section.

  1.  Obesity and Overweight:

Overweight and obesity are defined as the increase in abnormal or excessive fat accumulation that may lead to impair health.

Body mass index (BMI) is used to classify overweight and obesity in adults by using simple index of weight-for-height. The BMI is calculated as a person's weight in kilograms divided by the square of his height in meters (kg/m2).

Women with high BMI’s have high risk of ovulatory issues and response poorly to fertility treatment. By following the healthy diet plan the menstrual irregularities and polycystic ovarian syndrome can be prevented.

By 2030, IDF estimates that:

  • One adult in ten will have diabetes
  • Diabetes-related health expenditure will exceed to USD$802 billion
  • 37 per cent of all adults with diabetes live in the Western Pacific region (which includes Australia);
  • China with over 100 million people with diabetes (ranked highest number of people with diabetes),
  • Indonesia with 10 million people with diabetes (7th highest),

Diabetes is the epidemic of the 21st century and the biggest challenge confronting Australia’s health care system.

  • 280 Australians develop diabetes every day. That’s one person every five minutes
  • Around 1.7 million Australians have diabetes. This includes all types of diagnosed diabetes (1.2 million known and registered) as well as silent, undiagnosed type 2 diabetes (up to 500,000 estimated)
  • More than 100,000 Australians have developed diabetes in the past year
  • For every person diagnosed with diabetes there is usually a family member or carer who also ‘lives with diabetes’ every day in a support role. This means that an estimated 2.4 million Australians are affected by diabetes every day

DIABETES IS INCREASING

Diabetes is the fastest growing chronic condition in Australia; increasing at a faster rate than other chronic diseases such as heart disease and cancer. All types of diabetes are increasing in prevalence:

  • Type 1 diabetes accounts for 10% of all diabetes and is increasing
  • Type 2 diabetes accounts for 85% of all diabetes and is increasing

The population of Australia usually comprises of 24 million (approximately estimated to be 25,300,200 million in 24th march 2019). It is the 52nd most populous country in the world. It comprises of 6 states and 8 popular cities. The most populated cities include Sydney (4.8 million), Melbourne (4.4 million), Brisbane (2.2 million) and Perth (1.9 million). According to the survey, it was found that Sydney was highly prone to diabetic disease from the age group of 55-75 years with the approx. population percentage of 69%.  The other cities are less prone to endocrine and diabetic diseases.

Type 1 diabetes often starts at a young age and is due to a lack of insulin. It accounts for 10-15% of all cases of diabetes. Type 2 diabetes starts with resistance to the action of insulin and is associated with older age, overweight and obesity.

Type-2 diabetes is much more common at older ages and the increase in the number of older people in Sydney also contributes to the increase in numbers of those with diabetes.
